WAIVER OF LIABILITY/PARENTAL CONSENT TO PLAY

 “I, or in the case of a minor, the undersigned parent or legal guardian of the above-named player, do hereby consent and agree that he/she may participate in the baseball program of the Optimist Club of Southwest London Inc. (also known as Southwest London Baseball “SWLB”). In exchange for my/my child’s right to participate, I agree to hold harmless and waive any and all claims of negligence against SWLB, its directors, coaches, volunteers, officials or any other person associated with SWLB with respect to my/ my child’s participation in the SWLB baseball program including but not limited to games, practices and travel to and from such participation. On behalf of myself, spouse, heirs, estate and assigns, I am waiving my right to recover any damages for injury or death as a result of any negligence on the part of SWLB, its directors, coaches, volunteers, officials or any other person associated with SWLB.

I am also aware that SWLB coaches, official and volunteers are not trained in first aid or able to administer first aid or any medications in any situation. I am aware that SWLB does not provide or require trainers or anyone trained in first aid to be present at any games, practices or other events for SWLB. I authorize SWLB or any of its coaches, officials or volunteers to make any medical decisions necessary on behalf of my child if I am not present. I am also aware that SWLB does not warrant that any of its facilities, playing fields, equipment, etc. is free of allergens, including but not limited to, nuts. I am also aware that my/my child’s name and image (either via photograph or video) may be used by SWLB for promotional purposes,including but not limited to the news media, the SWLB website, individual team website or team sponsor plaques. I agree that both myself and my child will abide by all rules and regulations of SWLB including the Code of Conduct. I agree that any violation of the Code of Conduct may result in disciplinary action against myself and/or my child which could include suspension and or expulsion from SWLB. I consent to SWLB sending me emails both during the baseball season and after. I understand that I may ask to be removed from the SWLB email distribution list at any time.

I AGREE that participation in baseball has many serious risks associated with it including serious injury and death and I am voluntarily assuming any and all of those risks for myself/on behalf of my child.”
